To file a program discrimination complaint, complete the USDA Program Discrimination Complaint Form, AD-3027, found online at How to File a Program Discrimination Complaint and at any USDA office or write a letter addressed to USDA and provide in the letter all of the information requested in the form. To request a copy of the complaint form, call (866) 632-9992. We want them to have a happy and productive year. Only by working together with mutual support and respect can we accomplish this goal. Parents are encouraged to come and visit their child's classroom at any time, any day. Parental involvement in a child's education is one of the most important aspects to help kids succeed in school. Continuous Improvement Goals Reading: All students will demonstrate an increase of grade-level comprehension and vocabulary in all content areas. Math: All students will improve in algebraic concepts and mathematical number sense.
